The Hoarding Connection of Cuyahoga County is engaged in a first-of-its-kind effort to determine the incidence and prevalence of severe hoarding within a large geographic area (i.e., Cuyahoga County). The data collection form here is part of that effort. Mental health professionals, care providers, public safety personnel and others are being trained throughout the county to take part in the data collection effort.

Please note that this is a DATA COLLECTION FORM only. No service provider will be contacted with this information, nor should it be used for that purpose. It will be held securely with no identifying information given out.

We are continuing to collect data from throughout Cuyahoga County, but we need your help! If you have completed data forms, please return them to the Cuyahoga County Board of Health via secure fax to 216.676.1317.

HIPAA PRIVACY RULE AND PUBLIC HEALTH Guidance from CDC and the U.S. Department of Health and Human Services
Public health practice often requires the acquisition, use, and exchange of PHI (protected health information) to perform public health activities (e.g., public health surveillance, program evaluation, terrorism preparedness, outbreak investigations, direct health services, and public health research). Such information enables public health authorities to implement mandated activities (e.g., identifying, monitoring, and responding to death, disease, and disability among populations) and accomplish public health objectives. Public health authorities have a long history of respecting the confidentiality of PHI, and the majority of states as well as the federal government have laws that govern the use of, and serve to protect, identifiable information collected by public health authorities.
